FORMS OF ELECTION DEPARTMENT
Election Commission of India has devised various forms (as per law) related to electoral registration process, such as – inclusion of name, deletion of name, correction of particulars etc.
These forms can be downloaded from below mentioned link or you can contact your respective Master BLO (Booth Level Officer) for details. These forms are following –
1. Form 6: Application for inclusion of name in electoral roll. (Residence Proof, Age Proof with two recent photographs).
2. Form 6A: Application for inclusion of name of overseas electors. (Residence Proof, copy of passport for NRI with two recent photographs)
3. Form 7: Application for objecting inclusion or seeking deletion of name in electoral roll.
4. Form 8: Application for objection / Correction to particulars entered in electoral roll. (Details of entries to be corrected: Name, Age, Father’s /Mother’s/ Husband’s name, Sex, Address)
5. Form 8A: Application for transposition of entry in electoral roll. (photocopy of previous voter card and photocopy of new residence )
6. Form eci-epic-002: Application for issue of duplicate photo Id card (To be available in election department)
